Evergreen Nephrology

With a rapidly evolving care model and new partnerships, including a major contract launch slated for early 2025, Evergreen Nephrology needed a refreshed brand and communications strategy that could scale with their vision. As an organization operating at the intersection of nephrology, value-based care, and complex partnerships, their audience segments include physicians, patients, payers, and investors, each requiring a tailored message. Evergreen’s leadership recognized that their existing brand and digital presence no longer reflected the maturity and sophistication of their operations, partnerships, or patient experience. 

Our role

As a strategic partner, TCD was tasked with leading a complete rebrand, messaging overhaul, website redesign, and ongoing creative support. Our work spanned three major scopes: 

Rebrand strategy and execution 

  Using our proprietary “ONE” framework, we partnered with Evergreen to identify the single most important idea behind their brand and bring it to life through messaging, logo design, and brand architecture. We worked through a phased approach to deliver high-fidelity logo concepts, co-branding strategies, and platform-ready messaging. Our process also accounted for executive leadership alignment, committee review, and future board presentation milestones. 

Creative services and strategic support 

We embedded with the Evergreen team to support accelerated messaging development, redesign patient and partner collateral, and refresh business development materials like pitch decks and one-sheets. A flexible, hourly model allowed us to respond quickly to shifting priorities while maintaining brand consistency. 

Website redesign 

 We reimagined evergreennephrology.com to reflect the new brand, positioning, and messaging. The site was designed for clarity, accessibility, and segmented engagement, with dedicated pages for partners, payers, investors, and patients. Our WordPress-based approach balanced design freedom with CMS access and full ADA and SEO compliance. 

Impact

Our work set up Evergreen Nephrology for a confident relaunch aligned with their business growth and care delivery transformation. The new messaging platform brings clarity and cohesion across all communications. The rebrand strategy repositions Evergreen as a thoughtful, scalable leader in kidney care, while the co-branded system supports a range of partnership models. The redesigned website reflects the organization’s maturity and is built to grow with them while improving accessibility, performance, and SEO. Creative support materials, including refreshed welcome kits, one-sheets, and patient mailers, further reinforce the new identity across key touchpoints. Together, these efforts help Evergreen show up with clarity and credibility in a complex, high-stakes space.
